Didactic Cardio/Vascular Program and am prepared to meet these obligations if

Technical Standards: I have reviewed and understand the technical standards
necessary to successfully complete the Ultrasound Didactic Program, including the
ability to perform standard ultrasound views, operate equipment competently, maintain
prolonged arm positions, lift and move patients, and possess adequate visual and
auditory acuity. I certify that I can meet these technical standards, with or without
reasonable accommodation. On this application, I have declared my area of interest. If
accepted into the program, I agree that no part of the course material will be copied,
shared, or transmitted to any other individual.
