Drug Screening: I understand that I will be required to undergo and pass drug testing
as a condition of clinical placement and continued enrollment in the program. I also
understand that drug screening may be conducted again at any time throughout the
program.

Background Check: I understand that I will be required to undergo and pass a criminal
background check as a condition of clinical placement and continued enrollment in the
program.

Clinical Placements: I understand that clinical sites may have additional requirements
(including fingerprinting, additional TB tests, and additional drug screenings) that will be
required for participation.
